BLOOD PRESSURE SCREENING: Free service provided by John Muir Senior Services.

CARE MANAGEMENT PROGRAM: Our Care Management Program provides resources and services to help seniors remain living independently, safely and with dignity for as long as possible. This program is partially funded by a Contra Costa County Community Block Grant.

CAREGIVERS' SUPPORT GROUP: A support group for those who function as caregivers. Group is limited to twelve participants. Call for more info.

FREE BREAD: Never fear, our free bread (and other items) will continue during construction however it will be located at a couple of different locations. Please take note: the days and times listed are exact. Bread will not be put out early and all remaining goods will be removed promptly at the end of the time slot.

This service is provided through the City of Pleasant Hill not the Pleasant Hill Recreation & Park District / Senior Center. Volunteer Drivers needed!!
